Our Approach

At Hope Mountain Nursing Services, we provide consultation and education services to our clients and their caregivers to help them plan for later life. A master's-level nurse with medical knowledge and experience meets with you to develop a comprehensive long-term plan tailored to your specific needs. Our collaborative approach involves identifying what’s important to you and developing a strategy that reflects your values while considering your medical condition and available resources.

The Hope Mountain Journey: 4 Key Steps

  • 1. Free Initial Consultation

    During your initial phone consultation, we'll discuss your situation and needs, both current and anticipated, as well as any concerns you may have. The consultation lays the groundwork for your meeting with the nurse advocate.

  • 2. Nurse Advocate Meeting

    Before your in-person meeting, the nurse reviews your medical records. She'll discuss your medical situation, answer questions, compare options for long-term healthcare and housing, and provide referrals to community resources.

  • 3. Written Plan & Report

    After your meeting, the nurse advocate will prepare a report that includes your long-term healthcare plan. We'll send your report to you so that you can also share it with key people, such as loved ones, financial advisers, and attorneys.

  • 4. Future Appointments

    As an established client, you may schedule future appointments to re-evaluate your care plan after a change in your medical status or to schedule periodic appointments with your nurse advocate to update the plan as needed.

Why do I need a long-term plan?

There are many care options as we age, and it can be confusing to compare them with their associated costs, as well as the pros and cons. Having a medical professional who can assist with navigating these options while anticipating age-related changes is advantageous before a health crisis occurs. 

Aging at home (with or without adult children or caregivers), independent living in a senior living community, assisted living, skilled nursing, memory care, and care homes are all choices that involve many factors.

And whether to utilize palliative or hospice care at some point in any of these housing situations is another important consideration.

Your nurse advocate is familiar with local options and can help you choose the type of situation you can afford and what will work best for your needs.

Meet Our Nurse Advocate

Cari Radley MSN, RN is Hope Mountain’s Owner and Nurse Advocate. She’s witnessed firsthand how rapid changes in the medical system have affected seniors, their living situations, and their care.

Cari has the desire and experience to assist people navigate complex situations for the best possible outcome. Her clinical specialty is palliative care, which seeks to improve quality of life and provide relief from pain and other symptoms of chronic illness.

Cari Radley has extensive experience in acute care, hospice, and palliative care settings, and is dedicated to providing compassionate and comprehensive services to her clients to enable them to live to their fullest potential as they age. Her bedside hospice nursing experience gives her a unique perspective on disease progression, family dynamics, and finding meaning at every stage of life.

As a visionary leader, Cari has successfully established and directed a palliative care patient program, developed a nurse coaching program for palliative care, and created and taught a course about female health, first aid, and nursing assistant skills for female university-age students in Cambodia. She’s also owned an agency for training caregivers.

Cari holds a Master of Science in Nursing Education and a Bachelor of Science in Nursing. Her multi-state nursing license allows her to practice in North Carolina and other participating nurse compact states.

An avid reader and lifelong learner, Cari also enjoys gardening, traveling to other countries, and walking along the trails in the woods near her house.

  • Your report will include a synopsis of your current medical situation and anticipated healthcare needs. It will also include a plan for your desired housing situation as your life progresses through stages where you may need different levels of care. A list of specific community referrals will also be provided for your current needs as well as future needs.
